Municipal Drug Alliance Survey
HELP WANTED: The Alliance is in need of your voice about Bloomingdale. Everyone is qualified and will be utilized. Simply click the link below and your voice will be heard.
Attached is a link to a survey that will provide valuable information that will be utilized in applying for a grant to help the Alliance help the community of Bloomingdale. It's 6 questions and will only take a few minutes of your valuable time.
The survey is anonymous and will only be used for the purposes of this application.
